This kit allows us to place implants immediately after extracting a tooth, providing efficient and effective treatment for our patients.

The Importance of Precise Placement

When it comes to dental implants, precision is key. With the Septum Drill Kit, we start by marking the bone in order to ensure accurate placement. This prevents any displacement of the drill or fixture during the procedure. It’s essential to preserve the integrity of the surrounding bone and ensure that the implant is in the correct position.

Extracting the Tooth with Care

Before proceeding with the implant, we carefully extract the problematic tooth. Our primary concern is to avoid any damage to the surrounding bones. In cases where the tooth doesn’t come out easily, we may need to section it for safer removal. Fortunately, this particular extraction went smoothly, and the bone structure was well-preserved.

Achieving Ideal Implant Placement

Once the tooth has been extracted, we proceed with the implant placement. Using the marking drill, we mark the bone to indicate the ideal position for the implant. It’s important to note that the implant doesn’t have to be drilled directly into the septum. Instead, we use the trephine bur at the marked spot to create a 3mm width hole. This is then widened to 4mm, followed by further drilling to accommodate the size of the fixture.

Inserting the Implant with Precision

One notable aspect of using the Septum Drill Kit is the unique feeling of inserting the implant. Unlike traditional methods, where the implant follows the extraction socket’s tilt, the implant is placed straight into the osteotomy site. This technique closely resembles inserting an implant into a fully healed ridge. In this case, we inserted the implant approximately 4mm below the buccal gingiva level to achieve the ideal emergence profile.

Ensuring Successful Healing

To promote successful healing, we take several additional steps. First, we graft bone in any gap larger than 2mm. This helps to fill in the space and support the implant. Additionally, we use allogenic bone and chip bone collected earlier, mixed with CGF, to facilitate easier manipulation. We then cover the site with a PRF membrane to aid in healing and prevent leakage of the grafted material.

A Final Touch for Healing

To complete the procedure, we perform a crisscross suture to adapt the gingiva closer to the healing abutment. This aids in the healing process and ensures a secure fit. A figure of eight sutures is also placed to further adapt the gingiva to the abutment, promoting optimal healing by secondary intention. With the flapless technique used in this case, minimal tension is applied during suturing to avoid any unnecessary complications.

Successful Outcome

Radiographs taken after two weeks reveal that the implant was placed in the middle of the septum, at the ideal depth and position. After removing the sutures, we observed that the soft tissue was healing well. Four months later, an impression was taken, and the abutment was fitted. The healthy gingival tissue around the abutment demonstrated the success of the procedure. Finally, we fitted a SCRP type Zirconia crown to complete this remarkable transformation.
